Facilitator Student Material Order Form

Dear Wonderful Facilitators,

Congratulations on Scheduling a 5 GIFTS Course.

The cost of the 5 GIFTS materials is $45 per student set, which includes the book, workbook and CD. To cover increased shipping costs you may want to charge your students $55-60 for the Course materials. 

If you are interested in offering your Course for a Class Fee and no royalty back to 5 GIFTS Ministry, please call Vivianne.

Note: In order to assure that you receive your materials on time, we require you to place your materials order 3 weeks prior to your first class. Missing this deadline will result in higher shipping costs.

It is no longer necessary to provide 5 GIFTS with the Letter of Agreement with Sponsoring Organization. However, please confirm that the Sponsoring Organization agrees to pay 5 GIFTS Ministry a 10% Royalty on all Tithes collected during the Course by checking “YES” below after 10% Royalty.
